General Document Submission Guidelines

• Print out, complete, sign, and bring the required form(s) with you.
• Bring a valid photo ID to verify your identity.
• Incomplete or unsigned forms may delay your assistance.

 If someone else is picking up food on your behalf, they must: 

• Fill out and sign the proxy form.
• Have both your signature and theirs on the form.
• Bring valid photo IDs for both the recipient and the proxy.
